At Ogden Preparatory Academy, we are committed to providing a safe, respectful, and supportive learning environment for every student.

If you have witnessed or experienced bullying, harassment, intimidation, or another student safety concern, please use this form to let us know. Reports may be submitted anonymously, and all concerns are reviewed by school administration as promptly as possible.

Please provide as much detail as you can so we can investigate the situation appropriately. If someone is in immediate danger or there is an emergency, notify a staff member immediately or call 911.
